Since joining the club in 2023, the Newport-born batter has made 96 appearances across all formats and become a key figure both on and off the pitch.
Adam grew up in Rookley and played for Ventnor Cricket Club before making his first-class debut.
He has scored more than 2,400 runs in first-class cricket for Worcestershire, including two centuries and 14 fifties.
The deal will keep him at the club until at least 2029.
Adam, formerly of Somerset and Warwickshire, said: "Any form of commitment from both sides is great and it was a very straightforward one to complete given how much confidence I got from the Club and the management and hopefully over the next few years I can repay that faith they have shown in me.
"The one thing that stands out here is the family orientated feel and how everyone knows one another, which I think translates into our cricket.
"We fight for one another and I think that is really obvious when we play together.
"This team has real potential in all three formats and there is no reason we can't be competing on all fronts, which is a really exciting feeling heading into next year."
Hose also captains the club's T20 side, the Rapids.
One of his standout performances came during the 2025 season, when he scored a career-best 266 against Hampshire in a record 395-run partnership with Jake Libby.
His T20 record includes seven half-centuries, with a top score of 95 from 51 balls against the Notts Outlaws last June.
Ashley Giles, Worcestershire CEO, said: "Adam has grown into an integral part of our side here at New Road, whether that be in red ball or white ball cricket, and is a really senior figure within the dressing room.
"We speak a lot about values and our culture, and he epitomises everything we strive to be both on and off the pitch, so to see him sign a three-year deal is a real statement for the Club."
Hose has also played in The Hundred and leagues in Australia, New Zealand and the Caribbean.
