The WXV Global Series match between Japan and Wales was stopped in the second half after a serious injury to Freya Bell.
Play was halted for more than six minutes, with medical staff from both teams treating the Wales star on the pitch at Prince Chichibu Memorial Stadium in Tokyo.
On 52 minutes and with Wales leading 7-0, centre Bell went down clutching her leg after a clear-out at a ruck.
Play initially continued around her as she lay on the turf holding her limb in obvious pain, with a Wales medic attending to her. The official eventually brought a break in play, which allowed Bell to be treated.
"Freya Bell looks in some real discomfort there," BBC Wales commentator Gareth Rees Owen said.
"Her right knee I think? Oh no. Just a horrible sight.
"We may be a while here now."
He added: "Our concerns are now with Freya Bell. Oh goodness."
Co-commentator and former Wales international Philippa Tuttiett added: "They do seem to be isolating that knee. Such a young player as well."
Wales co-captain Alex Callender, on the occasion of her 50th cap, could be seen comforting her team-mate as the medical team looked to ease the pain Bell was in.
"Absolutely not (good to see)," Tuttiett added as three medical staff worked on Bell's leg, who would later be joined by the Japanese support staff.
"Especially for the players left on the field, seeing your team-mate go down like that."
Eventually, after a significant stoppage, Bell left the pitch on a stretcher, in what will be a huge concern for Wales head coach Sean Lynn.
Owen continued: "It's so nice to see Alex Callender there supporting her."
Courtney Keight replaced Bell.
Wales went ahead in the match thanks to Seren Lockwood's opening try, which was converted by Bell in the first half.
